In Med's link, they suggest mandatory covers for elevator carriage openings (the "hole" that the waste bin hits the top of).
I live in Sweden, and I don't really know what kinds of elevators they use... at least I never see any stuff moving when I use the ones here, the doors close and then... biing! Maybe some hospital?

To clarify: I don't know any places here that use these "problem elevators where floor after floor pass an unprotected opening" except maybe hospitals.
